Appointments
Initial appointments are scheduled for a 50 minute session. Subsequent sessions are scheduled for a 45 minute session. Appointment times include the time spent completing any necessary paperwork. Arriving on time is important to insure you receive the best possible service.

For your convenience a short Client Intake Form is available for download. If you have an Intake appointment scheduled please complete this form and bring it to your appointment.
If you need your health information released to a third party please notify at the time of your visit.

Cancellations
Please contact us 24 hours in advance to cancel or reschedule an appointment. No shows and appointments cancelled with less than 24 hours notice will require full payment.

An appointment can be scheduled at a time that is convenient for you. Evening and weekend hours are available.
